Care management services refer to the ongoing coordination of all aspects of a person’s healthcare, especially when they have complex or chronic needs. At Haven Healthcare Advocates, this means a dedicated registered nurse oversees appointments, medications, communication with doctors, facility visits, and more. It’s a hands-on, personalized service designed to help families stay informed and ensure nothing falls through the cracks particularly for aging adults who may need extra support managing their health.

Medicare does cover Chronic Care Management (CCM) services for eligible patients with two or more chronic conditions that are expected to last at least 12 months. These services are usually provided by a physician’s office or healthcare provider and include things like care coordination, medication management, and regular check-ins.
At Haven, while we don’t bill Medicare for CCM directly, our private-pay care management services go beyond standard Medicare offerings providing personalized support, advocacy, and ongoing coordination with healthcare providers, specialists, and facilities. We’re also able to help families understand what Medicare does and doesn’t cover so they can make more informed decisions.
Because we are private pay we work directly for them and for their benefit!
